Before your visit
A short list, so the first few minutes are easy. Bring what you can, and if something is missing the office will help you sort it out when you arrive.
Six things to have with you
Arrive 15 minutes early
That leaves time to check in without rushing, and to ask the front desk anything before your appointment starts.
Bring your insurance card
The office needs it at check-in. If your coverage changed since your last visit, bring the current card.
Bring a government-issued photo ID
A driver license, state ID, or passport is fine.
Bring a current list of medications
Include prescriptions, over-the-counter medicines, and supplements, with the doses if you know them. A photo of the bottles works.
Bring recent labs or imaging, if you have them
Any results you already have on paper or on your phone can save a step. If you do not have them, that is fine — the office can request records.
Ask about preparation when you schedule
If your visit involves a procedure that needs special preparation, the office will give you specific instructions at the time you schedule.
Not sure what applies to your visit? Call the office at(941) 246-2700 and ask.
